Complaint Resolution Procedure

Except as otherwise required by applicable state law, the following is the Program’s resolution procedure:

If for any reason you become dissatisfied with the level of service provided by the Program, you may contact our Customer Service Department, toll-free, at: 800-441-0380.

The Program’s cardholders always have the option of filing a complaint or asking any question in writing. Please address your inquiries to:

When submitting your inquiry please include the following:

  • Your name, address and telephone number
  • The details surrounding the reason for your inquiry or complaint
  • Information concerning the efforts that you have made to resolve the matter
  • All responses that other parties have made in response to your complaint
  • How you would like to see that matter resolved
  • Nationwide DMPO Disclosure for Free Rx Plans

    THIS PLAN IS NOT INSURANCE and is not intended to replace health insurance. This plan does not meet the minimum creditable coverage requirements under M.G.L. c. 111M and 956 CMR 5.00. This plan is not a Qualified Health Plan under the Affordable Care Act. THIS IS NOT A MEDICARE PRESCRIPTION DRUG PLAN.* The plan provides discounts at certain health care providers for medical services. The range of discounts will vary depending on the type of provider and service. The plan does not make payments directly to the providers of medical services. Plan members are obligated to pay for all health care services but will receive a discount from those health care providers who have contracted with the discount medical plan organization. You may access a list of participating health care providers at www.medvana.com/medvana. Upon request the plan will make available a written list of participating health care providers.
